When referring a patient for total patient care, ultrasounds, consultations, antenatal testing, and/or diagnostic testing, please fill out the referral form below. Fax the referral form and all relevant medical records to our office so we can schedule your patient promptly. Our referral form outlines required information for accurate and complete service scheduling. Do not hesitate to contact our office for clarification as needed.

Please note, total patient care transfers are approved by the physicians on a case-by-case basis. Deciding factors include medical indications and physician availability at the time of delivery.

For Patients

Our providers specialize in Maternal-Fetal Medicine, which means they only provide care for high-risk pregnancies. Most pregnancies are confirmed by general OB/GYN physicians before patients are transferred for care at our facility. Typically, the general OB/GYN physician determines that the pregnancy is more complicated and requires specialty care with one of our providers. In these circumstances, your general OB/GYN physician sends a transfer of care request along with your medical records to our practice. Once these forms are received, our office will contact you to schedule an appointment with one of our Maternal-Fetal Medicine physicians.

Occasionally, patients with a complicated pregnancy history or current medical problems can self-refer for obstetrical care at Scottsdale Perinatal Associates, LLC. Our providers will review your records and your high-risk pregnancy indicators. After assessing this information and checking provider availability, we will call to schedule your appointment or advise you to seek care with a general OB/GYN physician. Please note, Maternal-Fetal Medicine physicians are specialists, so our billing practices are different than general OB/GYN providers and may incur more costs for patients.
